Your first visit to Comeau Health Care Associates will give you an opportunity to find out if chiropractic care or physical therapy can help you. You will receive a thorough case history and examinations to make sure that your condition can be helped through the natural healing of chiropractic adjustments.
Please click on the appropriate link below, print the forms, complete them, and bring them with you to your appointment, with your driver's license and Insurance card.

At Comeau Health Care Associates, we believe in naturally healing, including chiropractic treatments, physical therapy, massage therapy, and nutrition.
On your initial visit, one of our doctors will provide a consultation, complete health history, examinations, and x-rays (if needed). Please allow 50-60 minutes for your first visit. We ask that you complete your new patient paperwork before your scheduled appointment time. These forms can be printed from the, “New Patient” link at the top of this page. If you are unable to have these completed we ask that you arrive 30 minutes prior to your appointment time.
The doctor will review your findings in great detail and discuss them with you and go over the proposed schedule of care. No treatments will begin until you understand what is being recommended and agree with this report of findings. You can expect service not pressure.
